Dr. Bageria who used to be at 3348 Keele Street used to be Scott Gallagher's family physician many years ago.
While he had his family doctor in 1999, Mr. Gallagher went into treatment for addiction to cocaine, alcohol, crack, and other illegal drugs, non- prescribed drugs, including hallucinogenics.
Mr. Gallagher was formally diagnosed with bipolar in 2004, and he was getting therapy prior to then. And he only went to the psychiatrist after the therapy did not work.
Mr. Gallagher seeked the opinions of three different doctors to confirm the diagnosis to be true. And ALL three doctors signed off on the bipolar diagnosis.
I had to take medication along my journey, lithium, bipolar ones, because I couldn't afford it.
And as a self-employed person, I made the decision to get off that medication, considering the damage to the liver and all.
The lithium does deliver over time. I've been able to function periodically, kind of like with bipolar. And that's been my struggle of survival as a self-employed person. I also suffer from mood swings, the really highs and the extreme lows.
Frank Brooks was the name of the doctor that I saw who was treating me for bipolar and he was giving me my meds for the bipolar and everything, all my meds, Concerta, depression, all related medications.
And then he died sometime after I got into the landmark forum about a couple years later. And then when Dr. Frank Brooks dies, I had an episode occur when I was still living at 40 Wickstead Way at the time. And somehow, my previous girlfriend whom I was living with at the time, Cindy Harris and my friends did an intervention on me and got me into CAMH, I believe that's what happened to the best of my recollection. My friend, Nafisha Somani who has known me for over 20 years, and knows of my situation, I am confident she will remember the exact details. What I am absolutely certain of is It was a real episode.
When Mr. Gallagher was at the peak of his school programs, helping at-risk kids in over 200 schools all over US a Canada, on a number of occasions when he was unable to carry on with his mission because of financial and economic hardship, Mr. Gallagher became suicidal.

I was only able to see him because I got in on a study. Dr. Jan was conducting a study of which I was a recipient of. The study had to do with no pharmaceutical drugs at all, that was what Dr. Jan taught for people, and it worked for a number of people and didn't work for a number of others. In terms of it being a solution to ADHD that did involve taking medication, I told him I didn't want to be on meds, but he convinced me to take them, and so I tried the meds in the ways he talked about, and I finally found my balance with the meds with six pills a day.
And I did go through bouts of depression right after Dr. Jan (then have to date the year) had to terminate me as a patient because he had accepted a new job where he was only allowed to work with children with ADHD. So he had to put all of his patients back into the care of their family doctor. So he wrote a note to a doctor when he sent me to him saying that I was doing well. And that may or may not be the case , I won't argue that.
But after I didn't have those meetings with my psychiatrist, Dr. Jan anymore, I had stopped running which was a habit for me sometime right after within the next six months as I lost my support system.
And I went through various levels of depression. And this was all while I was building All Addictions Anonymous, which was very successful worldwide helping 1000s of addicts and addicts helping addicts all over the world.
Then when COVID happened, he had to shift his focus and attention on his roommate, Sameena Fernandes, and help her with her mental issues, such as anxiety, depression, suical ideations, etc. And I'm still her caretaker.
I also want to note that on this date, 2003, Scott went bankrupt caused by the effects of ADHD. His inability to focuss and not being able to do his basic bookkeeping, and being scattered brain and experiencing extreme overwhelm, and easily distracted, which eventually resorted to claiming bankruptcy for the first time in 2003 and then for the second time in 2018 because of his ADHD illness.
So in 2003, the impact of his illness caught up with itself to the point where he had to claim bankruptcy.
And then fortunately for him, while helping all these addicts, he had already founded All Addictions Anonymous and was still helping people all over the world.
And he still dedicates two hours a day over zoom to do his pro-bono work. And Sameena Fernandes confirms this to be true for at least the last five years and this is a signed affidavit.
Furthermore. In getting the official diagnosis in 2004 for ADHD, bipolar, seasonal affective disorder, and (whatever else was mentioned in that letter) and all the medications that were recommended by the doctor to get of which he couldn't continue to afford throughout his life as a self employed person.
Therefore, he was only able to be emotionally and mentally stable when he was not impacted by financial and economic hardship that would have him not being able to afford his medications that were absolutely required for him to, for one have the capacity to function.
So since (2008 - Check with this is the correct date), he's been on his ADHD medication.
In 2010, Ms. Fernandes met Scott in a mental health course, called Mental Health Assessments. At the time, Mr. Gallagher was in the peak of doing his school programs.
As the two of them got to know each other while in the course, studying on various course projects together, Ms. Fernandes got invited to volunteer on one of Scott's not-for-profits, called Addiction-Free-Youth which helped helped kids struggling from various addictions.

On countless occassions, Ms. Fernandes witnessed Scott being unfocused, overwhelmed with little tasks that would not be overwhlming for an average person to do, he would be constantly losing things up to the point where Sameena had to take on his accountability to help him not lose things. He would lose things such as his glasses, phone, keys, etc, etc. And Sameena witnessed him also lose his medication at one point. So Scott asked Sameena to be responsible for his own well-being with respect to help him not lose things.
And Sameena can testify that throughout 2015 Until today, 2022, Scott has constant difficulty following through on all his many amazing ideas and completing anything; this is documented in all the downloads that he recorded, he would get so many ideas in his morning meetings with Sameena that didn't get completed.
So now Sameena is again supporting Scotts weaknesses, ADHD deficiency by being somewhat of his assistant, performing such tasks to support him like taking notes and in managing Scott's accountabilities by just talking to him weekly just so he is fully supported .
And once a week over their weekly zoom session, Scott consults with Sameena about how he can support her and how she can support him. So apart from beng roommates, this is the dynamic of their relationship.
Therefore, Scott is also making sure that he's supporting Sameena with her mental and emotional well-being and her business. He acts as her coach, so she can still make some progress while she's going through debilitating mental and emotional episodes that occur daily.
So at least she's still kind of moving forward with her business, passion projects, such as Get Booked To Book Others with Scott's assistance and guidance. So they are acting in the capacity of caretakers for each other.
And to be clear, both Scott and Sameena said under oath, that they've never had sex, and that is not the nature of their relationship, especially with the difference of age and sexual preferences.
Sameena confirms that the other reason Scott also went bankrupt apart from his various diagnosis, such as ADHD, he put his business on hold and focussed on Sameena's well-being with supporting her after she lost her job and career in 2017. Scott represented her for that year winning her $30,943.71 For the record for all the back wages that her previous employer did not pay her for the overtime hours, 60+ she was working, and Scott did not get a penny of her winnings. Even though, Sameena offered, Scott would not take a penny of it, because that was not the nature of their relationship. Nor is it now.
So Sameena just confirmed that as her understanding of bipolar, ADHD by researching it on the internet, and talking to the doctor, she's very, very clear that Scott's ADHD and bipolar symptoms, she witnessed them being present, at least 90% of the time. And she would witness his mood swings even while they were on video, such as zoom. She would witness him being at the highest almost like mania every morning. But then when he gets depressed, it happens so quickly, and it's very difficult for him to get back out of it.
By the time he gets back out of it, he's not present to all his commitments that he he made and he forgets all his priorities. It's like he loses his focus entirely. And in my opinion, that's why he went bankrupt. And in my opinion, it's also because he could not afford his meds , other than his ADHD medication which he could also barely manage to afford. but just eat but despite the ADHD medication.

Thank you so much for treating my situation and that of Ms Sameena Fernandes will such urgency.
I have replied to Sameena’s email to you and I have also cc’d Dr. Goldberg at wellmedicareception@gmail.com should you wish to confirm anything with him.
Just letting you know that, ive been self-employed for about 30 years now and I never expected I would be asking for this kind of help.
Back in 2004 I was diagnosed with Adhd, depression and bipolar 2. I went bankrupt in 2019 and then my financial/business situation worsened even more when lockdowns started happening March of 2020.
As Sameena Fernandes’ roommate, I gave her my legally required 60 days notice to terminate my room mate arrangement with her due to landlord harrassment as per this video https://youtu.be/DujP_SZEJxc I sent to Sameena.
I also have a business/wellbeing hybrid agreement that I created for Sameena when she first confessed to me on April 4th 2017 that she wanted to end her life. You can see what Sameena wrote about that agreement on this page https://passionprojects.kartra.com/page/lWC488
Sameena responded to my video message to her of me giving my notice, by sending me this video https://youtu.be/7hX6jg4gysk a couple days later, where she made an offer for me to live rent free in exchange for me supporting her well-being during covid and represent her at the various LTB hearings against her landlord who was harassing her, and which a final T2 hearing date for me to represent Sameena has still not been set.

Anyways, when the lockdowns happened, both my primary doctor and Sameena’s primary doctor refused to see their patients in person so (to save on travel expenses and because sameena does not drive) we both saw a different doctor ie Dr Norman Goldberg a number of times together in back to back appointments primarily to support Sameena with her anxiety/paranoia condition I witnessed ongoingly.
This past Thursday, March 17th, we had another doctors appointment which was supposed to be for the purpose of Dr Goldberg doing the Trillium paperwork for me because my adhd disability has me overwhelmed in doing such paperwork - and because I can no longer afford my essential medications which also includes medications for gout and urinary problems.
Sameena’s appointment was originally supposed to be about Dr Goldberg helping Sameena submit paperwork for her disability.
Dr Goldberg told us during this appointment that both Sameena and myself qualified for some sort of a disability tax credit but when I told Dr. Goldberg during our appointment that I had to cancel my car insurance so that I could use my car insurance money to buy groceries, AND given our history from numerous appointments during covid, Dr. Goldberg knew how our financial situation had progressively gotten worse and worse over the past many months.
He therefore advised myself and Sameena that we both qualified individually for Social Assistance (since we are not in a domestic partnership arrangement) and that he would fill in the paperwork and submit both of our applications himself online for us during our most recent March 17th appointment with him which he obviously did.
Both mine and Sameena's appointments together ended up being more than 2 hours long because during that appointment, Dr. Goldberg read the application questions to us and typed in our verbal answers as he explained each of the questions to make sure we were not confused, and then he helped us read everything over before signing off on our answers and submitting.
I want to conclude by saying that I feel very embarrassed asking to be supported by the government in this way and that, should I qualify, my intention would be to NOT take advantage of this kind of assistance for more than 90 days.
Both Sameena and myself have found ourselves in a situation where we find ourselves financially "dependent" upon each other out of necessity so to speak - because we have no savings so if we were to get evicted, neither of us could afford first and last months rent separately so we need to stay together as room mates for our mutual survival and wellbeing.
As for the banking information questions you have asked for Amanda, I will send you another email tomorrow before 5PM with those details you have asked for related to me personally.

Back in 2004 I was diagnosed with Adhd, depression and bipolar 2. I went bankrupt in 2019 and then my financial/business situation worsened even more when lockdowns started happening March of 2020.
As Sameena Fernandes’ roommate, I gave her my legally required 60 days notice to terminate my room mate arrangement with her due to landlord harrassment as per this video https://youtu.be/DujP_SZEJxc I sent to Sameena.

Sameena responded to my video message to her of me giving my notice, by sending me this video https://youtu.be/7hX6jg4gysk a couple days later, where she made an offer for me to live rent free in exchange for me supporting her well-being during covid and represent her at the various LTB hearings against her landlord who was harassing her, and which a final T2 hearing date for me to represent Sameena has still not been set.

Anyways, when the lockdowns happened, both my primary doctor and Sameena’s primary doctor refused to see their patients in person so (to save on travel expenses and because sameena does not drive) we both saw a different doctor ie Dr Norman Goldberg a number of times together in back to back appointments primarily to support Sameena with her anxiety/paranoia condition I witnessed ongoingly.
This past Thursday, March 17th, we had another doctors appointment which was supposed to be for the purpose of Dr Goldberg doing the Trillium paperwork for me because my adhd disability has me overwhelmed in doing such paperwork - and because I can no longer afford my essential medications which also includes medications for gout and urinary problems.
Sameena’s appointment was originally supposed to be about Dr Goldberg helping Sameena submit paperwork for her disability.
Dr Goldberg told us during this appointment that both Sameena and myself qualified for some sort of a disability tax credit but when I told Dr. Goldberg during our appointment that I had to cancel my car insurance so that I could use my car insurance money to buy groceries, AND given our history from numerous appointments during covid, Dr. Goldberg knew how our financial situation had progressively gotten worse and worse over the past many months.
He therefore advised myself and Sameena that we both qualified individually for Social Assistance (since we are not in a domestic partnership arrangement) and that he would fill in the paperwork and submit both of our applications himself online for us during our most recent March 17th appointment with him which he obviously did.
Both mine and Sameena's appointments together ended up being more than 2 hours long because during that appointment, Dr. Goldberg read the application questions to us and typed in our verbal answers as he explained each of the questions to make sure we were not confused, and then he helped us read everything over before signing off on our answers and submitting.
I want to conclude by saying that I feel very embarrassed asking to be supported by the government in this way and that, should I qualify, my intention would be to NOT take advantage of this kind of assistance for more than 90 days.
Both Sameena and myself have found ourselves in a situation where we find ourselves financially "dependent" upon each other out of necessity so to speak - because we have no savings so if we were to get evicted, neither of us could afford first and last months rent separately so we need to stay together as room mates for our mutual survival and wellbeing.

I promised you yesterday via email that I would send you my banking information before 5pm today and am not able to do that. I am re-promising to get that to you before 5pm tomorrow instead.
Short explanation for this delay is that an unexpected financial crisis situation happened that took me 2 hours to deal with today. My car insurance payment that was not supposed to go through my bank account because I had put my car insurance on suspension last week actually went through my account last night and as a result I got dinged 45 dollars NSF with bank as well as another 50 dollars with the insurance company for NSF on their end ie even though the insurance company promised me that they would be putting no more than 33 dollars through on Monday through my account their system tried to put the full 186.83 cents through resulting in these NSF charges.
Anyways, it’s a long boring story but what I was able to do so far is create this password protected page for you

The auto insurance agent I previously told you I had spoke to, after escalating the call and talking to his finance team about my situation, advised me that their system will attempt to take out the 186.83 cents again as early as next Monday - and that there is nothing the agent can do to stop their next attempt at getting this payment, so if I don’t have the 186.83 in the account when they try and take it again, then I will be getting another 50 dollar NSF charge from them, which means I would get another 45 dollar NSF charge from my bank as well.
I’ve also been told that within the next 2 weeks my car insurance company will be crediting my bank account approximately 91 dollars. Bottom line is that, in reality, my simplii bank account has 261.01 minus 186.83 plus 91 dollars which equals 165.18
My intention for pro-actively putting my car insurance on suspension was so that I could have more money to purchase my meds and groceries and I currently have a prescription waiting at the pharmacy for me which costs over 150 dollars to fill and which is money I don’t have right now - and I still have 4 more days left of this prescription before it runs out.
I should have 113 dollars in my tangerine bank account but when I went in the account this morning it says I only have 13.16 cents available and it says that is because they’ve put a hold on 100 dollars of my money because the monthly bell internet fee is coming out of my account at midnight tonight.
After looking everywhere for spare change, as of right now I have a total of 27 dollars in cash as well.
I own a 2007 toyota prius which needs servicing and is parked downstairs in the garage until I can afford to activate the insurance again and I estimate the value of the car to be worth no more than 3 thousand dollars. My doctor typed 3 thousand in the form for me based on me telling him that figure which is probably a bit high. Maybe im wrong but I don’t think so.
